An interesting medical renal biopsy case which required electron microscopy to make an unexpected diagnosis.
An older patient with multiple co-morbidities presented with declining renal function. Histology and immunofluorescence gave conflicting pictures, neither of which fit particularly well with the history given or adequately explained the sudden decline in function.
Electron microscopy was requested and provided the information needed to make a diagnosis and explained the unusual IMF pattern.
