This session explores implementation experience in Scotland and Catalonia – two devolved healthcare systems that have embraced integrated health and social care policy and practice.
Speakers from Catalonia will share emerging insights and early impacts of ADMIT, a two-year technology-enabled demonstrator project involving 19 primary care and social care centres across Catalonia. The project seeks to scale up coordinated, comprehensive and integrated social and health care, aiming to reach up to 1000 persons with complex health conditions or social issues. The team will also highlight new legislation to drive forward health and social care integration across the region and will discuss the role of the Integrated Care Agency to support implementation.
Speakers from Scotland will reflect on a decade of implementation experience since enacting legislation to integrate health and social care. They will discuss key enablers of further progress - embedding lived experience in co-design and delivery of care and support, investment in prevention and community-led support, digital transformation and inter-professional and cross sector education on cultures and leadership for integration.
